CITY OF MIRAMAR 

RFP 08-08-03 
CITY GROUP  HEALTH PLANS  INCLUDING  RE-INSURANCE &  THIRD PARTY ADMINISTRATION

Pursuant to Florida  State Statutes,  Chapter 287, The City  of Miramar will  receive sealed  proposals to the  attention of the office of the City Clerk,  2300 Civic Center  Place, Miramar, Fl.  33025 until 2:00 PM.  TUESDAY,  SEPTEMBER 9TH,  2008

FOR:  GROUP HEALTH  PLANS

The City of Miramar  invites qualified  proposers to submit proposals,  statements of  qualifications and  experience to enter into an agreement  with the City of  Miramar for GROUP  HEALTH PLANS INCLUDING  RE-INSURANCE AND  THIRD PARTY  ADMINISTRATION  on a fully insured or  self-funded basis.

Proposers shall  submit ONE  MARKED ORIGINAL  AND SIX  PHOTOCOPIES for a  total of SEVEN  COMPLETE PROPOSAL PACKAGES, which  provides the required  information. The ORIGINAL should be  unbound, each  photocopy shall be  bound in a 3-ring  binder. The City has selected  Lloyd F. Rhode's of  the Rhodes Insurance  Group to act as their  exclusive Broker/Consultant in  reference to this  request. No broker or  agent participation is required.

The offers will be  publicly opened and read shortly after  2:00 PM September  9th, 2008 at the City  of Miramar City Hall  Lobby. The City Clerk's time clock  shall be the sole  judge of time. RFP's  submitted past the  deadline and/or submitted to any  other location shall  be deemed  non-responsive.

The City of Miramar  reserves the right to  reject any of all  offers, to waive any  informality in a  proposal received, to  re-advertise for  offers, to award in whole or in part to  one or more offerors,  or to take any such  actions as deemed in  the best interest of  the City.
