REQUEST FOR PROPOSAL: RFP NO. 08-102/DL
Palm Beach County Board of County Commissioners is soliciting sealed proposals from qualified proposer(s) for supplying:
TITLE: TERM LIFE AND AD&D INSURANCE
Date of Issuance: July 21, 2008
Proposal Due Date: August 20, 2008
PURPOSE: The purpose of this Request for Proposals is to
seek proposals for an employer paid, Board of County Com- missioners, Solid Waste Authority, Palm Tran, Inc., and Supervisor of Elections (hereinafter referred to as the "GROUP") Term Life and Accidental Death and Dismemberment (AD & D) plan and a voluntary, supplemental group term life and AD & D plan.
This latter voluntary plan will be paid entirely by the employee through payroll deduction and shall offer coverage on both the employee and dependents. Voluntary retiree coverage (to be paid by the retiree) is also required. The current employer paid benefit structures shall be duplicated by the proposer. The voluntary, supplemental employee paid benefit structure may be duplicated or enhanced by the proposer preferably at reduced or no additional cost per $1,000 of coverage selected. In particular, the GROUP wishes to consider the possibility of increasing the current maximum amount of voluntary benefits to a maximum of $300,000 for the employee, and additionally, would like to increase the amounts of voluntary dependent coverage as allowed by Florida Law.
The GROUP prefers to enter into a five year, fixed rate contract. The effective date of the contract is to be January 1, 2009. Guaranteed rates for a minimum period of two (2) or three (3) years, with annual renewals using rate caps based upon pre-defined formulas, may also be considered.

All proposals must be submitted in accordance with the instructions contained in the RFP document and must be received in the offices of the Purchasing Department no later than August 20, 2008, 4:00 p.m. local time. Envelopes must be identified by the closing date and the RFP number. CAUTION: Amendments to this Request for Proposal will be automatically sent only to those vendors who received this document directly from Palm Beach County Purchasing Department.
