RFP for Insurance

REQUEST FOR PROPOSAL

The City of Camden through the Department of Risk Management intends to acquire services for the following:

RFP 05-16 TO PROVIDE AN EMPLOYEE ASSISTANCE PROGRAM FOR
THE 1,200 EMPLOYEES OF THE CITY OF CAMDEN.

RFP 05-17 PRIMARY MEDICAL CARE, PRE-EMPLOYMENT PHYSICALS, COMMERCIAL DRIVER'S DRUG TESTING AND RETURN TOWORK EVALUATIONS, RANDOM DRUG & ALCOHOL TESTING.

RFP 05-18 EXCESS WORKERS COMPENSATION, GENERAL, AND AUTO LIABILITY, AUTOMOBILE PHYSICAL DAMAGE, PROPERTY INSURANCE, BOILER & MACHINERY, AND CRIME.

Guidelines for the Proposal may be obtained at the Office of the Purchasing Agent, Room 213, City Hall, Sixth and Market Streets, Camden, New Jersey.

The submitting person/agency must be qualified and experienced to conduct the services required in the Proposal. Those not providing written proof of required qualifications and experience will be disqualified.

If awarded a contract, the contractor will be required to comply with requirements of P.L. 1975, c127 (Law against Discrimination in Public contract).

DEADLINE FOR SUBMISSION OF PROPOSAL is by 3 p.m. on Tuesday, September 27, 2005 in the Office of the Purchasing Agent, Room 213, City Hall, Camden, New Jersey 08101-5120

Proposals will not be accepted after the time specified submission. Proposals received after the hour appointed will be deemed invalid and returned unopened.
